Fields that are marked with red asterisks (*) are mandatory.

Field Information

Title

Use the prompt to select from the list.

Surname*

The employee's surname must not contain spaces and can be up to a maximum of 24 alpha characters. If the surname is double barreled, use the hyphen to link the names, e.g. SMYTHE-SIMPSON.

First Forename*

The employee's name must not contain spaces and should only contain alpha characters.

Second Forename

The employee's name must not contain spaces and should only contain alpha characters.

Maiden Name

The Maiden Name is an optional field.

Address Line 1*

The employee's home address can be entered as up to 4 lines of address. The 1st and 2nd line of address is mandatory.

Address Line 3 and 4

These fields are optional.

Country

This field defaults to UK. Use the prompt to select from the list.

Post Code

Enter the post code associated with the UK addresses only.

Date of Birth*

Enter 'DD/MM/YYYY' or use the calendar to pick a date.

Age

This field displays the employee's age as calculated form the Date of Birth.

Gender*

Use the prompt to select from the list.

Marital Status

Use the prompt to select from the list.

National Insurance Number (NI)

Enter the number in the following format: AANNNNNNA (ex. AB123456C). The NI number must be present before the end of the tax year.
